4 Easy Steps!

1. Fill out online registration forms.
   
   
2. Fill out form completely. (If Greek get signature from OFSA.) If hosting the event off campus you will need to get the manager or owner’s signature from the club/bar/restaurant.
   
   
3. If hosting event at club/bar/restaurant, obtain a copy of the liquor license and liability insurance. Ask owner or manager for these documents – they can be faxed directly to Office of Alcohol & Other Drug Program Initiatives [fax: 215.573.7944]
   
   
4. ONE WEEK prior to the event, hand in completed registration form to Julie Lyzinski, Interim Director of the Office of Alcohol & Other Drug Program Initiatives, 3611 Locust Walk, Third Floor. Registration must be turned in by Monday at 2 PM.
   

** REMEMBER ** The organization's 'competency plan' must be handed in before the first registered event of the year.
