Registering Parties BothOn Campus and at Bars/Clubs/Restaurants

4 Easy Steps!

1. Fill out online registration forms.
   
   
2. Fill out form completely. (If Greek get signature from OFSA.) If hosting the event off campus you will need to get the manager or owner’s signature from the club/bar/restaurant.
   
   
3. If hosting event at club/bar/restaurant, obtain a copy of the liquor license and liability insurance. Ask owner or manager for these documents – they can be faxed directly to Office of Alcohol Policy Initiatives [fax: 215.573.7944]
   
   
4. ONE WEEK prior to the event, hand in completed registration form to Stephanie Ives, Director of Alcohol Policy Initiatives, 3611 Locust Walk, Third Floor

** REMEMBER ** The organization's 'competency plan' must be handed in before the first registered event of the year.
