How do I return from Medical Leave?

You can start the process by downloading the necessary forms which must be completed by your treating professional. Once that's done, the forms must be returned or faxed to Dr. Bill Alexander here at CAPS at 215-573-8966.

Dr. Alexander will review the forms, and may call you or your physician about your readiness to return to Penn. He then notifies your advisor about your ability to return.
