Procedure
Complete the appropriate on-line form (see below).
Fill in the form completely. No incomplete forms will be accepted.
Bring the form to the Office of Fraternity Sorority Life (3933 Walnut Street) to be signed by a program staff member during office hours, Monday - Friday 9:00 a.m. - 5:00 p.m.
Make a copy for your records.
Deliever the original form to VPUL (3611 Locust Walk) by 2:00 p.m. on the Monday before the event.
