Student Organization Registration Form 2008-2009

IMPORTANT: There will be a new registration system in place soon. If there's a reason you have to register before it's ready -- SAC and OSL are not mandating that -- you may use this form. More information will be provided here and via e-mail after Labor Day.

Note: Please wait to submit this form until you have all the names, addresses and phone numbers requested. Groups will not be added to the contact list until forms are complete.

*Important*
All undergraduate student organizations seeking University recognition are required to send two members of the organization to a health intervention training offered by the Drug and Alcohol Resource Team (DART). Organizations that do not comply with this mandate could be subject to a variety of consequences including: loss of recognition, loss of funding, loss of University privileges (i.e., room reservations, University email accounts, etc.). Use this online registration form to sign up for the training; a new window will open and you can return here after submitting your training form.

Membership in registered campus organizations must be open to all persons without regard to race, color, sex, sexual or affectional preference, religion, national or ethnic origin, handicap, or disability. Under Title IX of the U.S. Education Act Amendment of 1972, certain exemptions may be granted for intercollegiate and intramural athletics, fraternities and sororities, and musical groups based on vocal range. Members of all campus organizations must conduct themselves at all times in a mature and responsible manner.

The rights and property of all persons are to be respected regardless of time or place. Failure to comply with University, City, State, or Federal laws and regulations can result in appropriate disciplinary action. Members of campus organizations are expected to adhere to standards of conduct established by Divisions and Departments of the University.

The University is an association of equals who, in working together, comprise a scholarly community. Hazing is inconsistent with the goals and purpose of the University and is explicitly forbidden.

Please read the University Policy on Membership (above) before submitting this form.

All groups must complete the first part of the form. All funded groups, graduate and undergraduate, must complete the second part. Only SAC groups or applicants should complete the last part. The submission button is located at the end of the document.

DO NOT USE THE ENTER/RETURN KEY. PRESS THE TAB KEY TO MOVE FROM FIELD TO FIELD. THE RETURN KEY WILL SUBMIT THE FORM.

Check here if you are only updating a form already submitted this year.

Check here if your group is currently a member of the Performing Arts Council. If so, please provide the names of two student technical staff. (They need not be trained; we can train them).
