This Continuation Plan is only available to insured students who lose their eligibility for the Student Insurance Plan in force with Aetna Life Insurance Company through official graduation or end of academic program, and whose PSIP coverage will terminate August 31, 2008. This plan will be available to terminating students and eligible dependents as long as they were enrolled in PSIP for the previous six months. If you wish to continue coverage, you will have the option to take advantage of all of the benefits under the 2008-2009 plan except for access to Student Health Services. Under the continuation plan, you may see any in-network provider without the need for a referral from SHS.
Your application for this continuation must be completed and postmarked by September 15, 2008.
