Your health care is your business. We understand that information about you and your health is very personal and therefore we are committed to protecting this medical information as required by law. We create a record of the care and services you receive from the Student Health Service (SHS). This record is necessary to provide you with high quality care and to ensure we are in compliance with State and Federal laws.

Requesting Medical Records:
Requests should be submitted within a reasonable time to allow for duplicating and mailing as records cannot be copied at the time of the request. You must fill out an authorization for release of Medical Records. Release forms are available in the Health Service. There may be a charge for copying your records.

To Request Copies of Immunization Records:
Submit an authorization for release of medical records to other medical care providers. Student health will no longer release health history information to a 3rd party.  Medical records will be released to the patient who can then forward the information as needed. 

Fee:  Please include you Penn ID number on your check or money order
$5.00 per copy of immunization history
$20.00 per copy of entire record
