Non compliant students:

Please be advised that on October 13, 2008, students who are out of compliance
With the immunization requirements will be blocked from spring registration. You may
avoid this sanction through the following actions:

Immunization records must be submitted via a secure web site. If you have
submitted paper records but have not submitted these online, you must submit these
online as well. You can view your immunization history/compliance status, make an immunization appointment and submit your immunization records here.

If you have submitted your immunizations online, they will not become part of
your record until we receive official verification. This verification consists of a  copy of the document you used to fill out the online immunization form.This should be an official medical document, with a healthcare provider’s signature or stamp.
These documents can be faxed to us at 215 746 0909 or may be submitted to the
Immunization Office in Student Health Service, 3535 Market St. Suite 100. Our office hours are from 9 to 5 Monday, Wednesday and Friday. 10-5 Thursday.
 
Please DO NOT send original documents.
 
If you need to update your immunizations, you may receive the vaccines at
the Student Health Service or you may elect to see another health care provider
separately.  In this case, you will need to provide us with written proof of your appointment. You may schedule an immunization appointment at the Student Health
Service online at https://shs.upenn.edu/opencommunicator or by calling 215 746 3535.

 
If you have no records but know you have had some or all of the required
Immunizations, you may have a blood titer test which can prove your immunity to certain
diseases. If you wish to have this test at SHS you can do so online at https://shs.upenn.edu/opencommunicator or by calling 215 746 3535 You may also elect to have this done elsewhere, but your will have to furnish us with the test results.
